Library Account/Barcode Request Form

You will need a library account/barcode to use the "Your Library" feature of the Sage Library catalog and to access BMCC online indexes and databases from off-campus. These features are restricted to BMCC students, staff, and faculty. Your status will be verified before a library account number is issued.

Please complete the following items. Your library account number (also referred to as your library barcode or library i.d. number) will be emailed to you within three working days. Incomplete forms cannot be processed, please include all requested information.
